Start Date: ASAP - no later than 06/30

Guarantee: 36 hrs/wk

Assignment Length: 12 wks

Experience Required: 1+ yrs in SNF setting preferred

Staff Composition: 2 full time PTAs, 3 PRN PTAs, 2 full time PTs, 2 PRN PTs

EMR Utilized: NETHEALTH

Caseload/Patient Scheduling: 8 daily treatments; productivity expectation is 85%. Note: This may vary if there are new evaluations during a given day.

Schedule: Typically Monday to Friday, 8 AM - 5 PM
