Description
- Conduct comprehensive assessments of patients in inpatient and outpatient environments, devising customized treatment plans tailored to their needs
- Implement therapy interventions that emphasize mobility, strength, coordination, and day-to-day functionality
- Collaborate closely with a multidisciplinary team-nurses, physicians, case managers, and other rehabilitation specialists-to optimize patient outcomes
- Guide patients and their families through the therapeutic journey, focusing on their goals, discharge plans, and at-home exercise routines
- Maintain meticulous documentation of evaluations, therapies administered, and patient progress per facility protocols and regulatory standards
- Engage in collaborative meetings and care rounds to guarantee cohesive and comprehensive patient care
- Doctorate in Physical Therapy (DPT) or equivalent qualifications from an accredited institution
- State licensure as a Rehabilitation Therapist
- CPR certification is preferred
- Additional specialized certifications (e.g., neurological rehabilitation, geriatrics) are beneficial, although not mandatory
- Prior patient care experience is highly preferred
- Ability to support patients facing complex medical and mobility challenges
- Excellent verbal and interpersonal communication abilities
- Familiarity with digital recording and clinical documentation systems
