Position Overview

As a Transitions Specialist at TeleMate Health, you will play a vital role in ensuring seamless transitions for patients as they navigate their healthcare journey. This position involves coordinating care and facilitating communication between patients and our healthcare providers to enhance patient outcomes and satisfaction.

Key Responsibilities

• Proactively make outbound calls and manage intake processes for new patients.
• Communicate proper expectations to patients
• Communicate functionality and expected utilization of monitoring equipment
• Ensure accurate record management by obtaining and communicating relevant patient information, patient history, insurance details, and intake forms.

• Inside or outside healthcare sales, preferred
• Proficient in data entry and Microsoft tools
• Demonstrates a high level of customer service and interpersonal communication skills
• Contact center or other high call volume experience, preferred
• Highly detail-oriented
• Ability to multi-task in a fast-paced work environment
• Strong communication skills and a commitment to providing compassionate care.
• Ability to work independently and manage time effectively in a remote environment.
• High speed internet at home
• Private and quiet workspace at home where phone conversations are uninterrupted and PHI cannot be overheard.
• Strong desire to provide patient advocacy
• Healthcare experience, preferred
