Job Details

Assesses the psychosocial needs of terminally ill patients and their families/caregivers, communicates findings to the interdisciplinary team, and provides support in the patient's residence or alternative settings when needed. Conducts social evaluations, develops and implements intervention plans, and provides counseling related to stress, coping, and end-of-life issues. Serves as a liaison between patients, families, community agencies, and organization personnel. Participates in care planning, interdisciplinary meetings, quality improvement activities, and supports team members in addressing social and emotional aspects of illness and death. Performs additional duties as delegated by the Clinical Supervisor.
